This formula is:
X: (D * x + 1)

Here, X is the minimum focal length of your lens in meters, and the dioptrie number on the close-up lens.

Just a calculation example:
You have an objective that can focus from 55 centimeters and you use this close-up filter with a dioptrie of +5. In this case the calculation is:
0.55: (5 * 0.55 + 1) And this is the same as
0.55: (2.75 + 1) or
0.55: 3.75 = 0.146 meters, so 14.6 centimeters
